Forms for Deposited Students

Students that have submitted their enrollment deposit to Cornell will be sent a check list in early June directing them to complete all or some of the following forms. If you are unsure of what forms you need to complete, please contact of Office of Financial Assistance. Federal Direct Stafford Loans: If you plan on borrowing the federal direct stafford loans that were offered to you as part of your financial assistance award, you must complete Once on the site, sign in using your Federal PIN, in the Manage My Direct Loan box. Next, you will select the Complete Entrance Counseling and Complete Master Promissory Note links. Once these two steps are completed, we will be notified electronically, and process your Federal Direct Loan. Perkins, McElroy or Sherman Loan online Master Promissory Note. If you were awarded one of these three loans as part of your financial assistance award, you will be asked to complete a Master Promissory Note and loan disclosures online. Students have to be set up in the system before you will be able to log in and copmlete this form. We will notify you when you should complete this. Receipt of Electronic Notification: All incoming students must read Cornell's Electronic Notification policy and acknowledge that they have read the policy.
Cornell Outside Scholarship Form: This form must be printed and completed ONLY if you are receiving outside scholarships.
Once on the studentloans.gov site, sign in using your Federal PIN, in the Manage My Direct Loan box.
